If these resources did not answer your initial reason for contacting Ann Meehan at Create Wellness Center for Child and Adolescent Therapy, a response will be issued after February 3rd, 2025 in the order in which emails and voicemails were received.

- Please access your client portal HERE to:
- Access your Maternity Leave Plan which includes your last session date, next appointment in office, and maternity leave coverage plan
- Access and pay your balance
- Reschedule or cancel appointments

- If you are needing to re-engage in services I will begin answering emails after February 3rd in the order in which they were received.

- Any records requests received after September 26th will take up to 60 days to process.
- Records requests will be processed in the order in which they were received.
